My dentist says I need a gingivectomy to prevent my crown from falling off again. It fell off three times in the last seven months. The base of my tooth is now dark, and I am considering losing it. Shouldn’t my dentist know whether I needed a gum procedure before placing the crown? This is ridiculous. How do I know that I really need this procedure? Am I justified in asking for a refund? – Thanks. Kamille from Silver Spring, MD
Kamille,
Crown lengthening (gingivectomy) involves removing gum tissue and perhaps some supporting bone to expose more tooth structure, allowing for a better crown fit. While this procedure is sometimes necessary, a dentist must decide if it is advisable.
Your dentist’s recommendation for crown lengthening after multiple crown failures raises concerns. A skilled dentist should be able to assess the tooth’s condition and determine the appropriate treatment plan, including the need for crown lengthening, before initiating the procedure.
It’s possible to place a crown on a shorter tooth without lengthening successfully. Proper tooth preparation, involving a gentle taper and potentially small grooves, can significantly improve crown retention.
It’s important to note that crown lengthening is most beneficial in cases where the tooth structure is significantly compromised, such as when a large portion of the crown is missing due to decay or fracture.
Given the repeated failures and your dentist’s recommendation for crown lengthening, we suggest seeking a second opinion from a more experienced dentist. They can evaluate your tooth’s condition and provide a more accurate assessment of your treatment options.
A crown that repeatedly falls off is not a serviceable restoration, and you should not be held responsible for the costs associated with its failure. If your dentist is uncooperative, consider filing a complaint with the dental board and your dental insurance company.
